During registration, you’ll take a “health evaluation” in which we ask you some questions around your health profile — including any medical conditions you have, your medical history, allergies, or medications you might be taking. Once you place your order, we send your health evaluation to our partner provider network, where a physician who is licensed in your state of residence will review your information to determine whether they can approve you for a CGM prescription. If the physician has any questions about the information you've provided, they'll reach out to you.
